Clients will be educated on what to expect, average patient outcomes, and realistic goals. Side effects and/or risks of the medications will be discussed at the initial consult. Consents to treat are required. Any required medical intervention, caused by above medications, will not be the responsibility of Ageless Med Spa or the provider.
Contraindications to Ageless Med Spa prescribing these medications include, but are not limited too:
history of pancreatitis, history of kidney or heart disease, currently obtaining additional treatment for diabetes, personal/family history of thyroid cancers, multiple endocrine neoplasia, hypoglycemia, those actively trying to get pregnant, those currently pregnant/breast feeding, those under the age of 18yrs of age, allergy to ingredients of medication.
Insurance will NOT be billed or used for the coverage of these medications/labs or the services at Ageless Med Spa. Pre authorizations for medications will not submitted to insurance.
There will be a $50 charge for no-show or fail to cancel appointment within 24hrs. Fee must be collected before new appointment is made. Three no-show appointments may result in client termination.
